This project works in partnership with Monmouthshire Disability Teams to provide sessions to young people with disabilities, from the ages of 5-18 years
All the children will have been assessed by a Social Worker from the Child Care Disability Team and we can only accept referrals from this Team.
Sessions may be 1:1, 2:1 or in a group setting at specific venues within the local community. We also provide Play Schemes for children.
This allows the young people opportunities to enjoy activities of their choice whilst being supported to develop their social, communication and practical life skills.
Mae’r prosiect hwn yn gweithio mewn partneriaeth â Thimau Anabledd Sir Fynwy i gynnig sesiynau i blant a phobl ifanc rhwng 5 ac 18 oed sydd ag anableddau.
Bydd pob plentyn wedi cael ei asesu gan Weithiwr Cymdeithasol o’r Tîm Gofal Plant (Anabledd), a dim ond achosion wedi’u cyfeirio gan y Tîm hwn y byddwn yn eu derbyn.
Gall sesiynau fod yn rhai 1:1, 2:1 neu’n rhai grŵp, mewn lleoliadau penodol yn y gymuned leol. Rydym yn cynnig Cynlluniau Chwarae i blant hefyd.
Bydd hyn yn rhoi cyfle i’r bobl ifanc fwynhau gweithgareddau o’u dewis, a byddant yn cael eu cefnogi i ddatblygu sgiliau cymdeithasol, sgiliau cyfathrebu a sgiliau byw ymarferol ar yr un pryd.
